1. Make an appointment either by phone 225-313-4560 or on-line (by clicking the Appointment Request link) 
2. Look for an email containing your required paperwork.
3. Videos are great but better picture quality is available on the camera feature of your phone. So take some great pics and text them to
225-313-4560 (dont worry; it’s encrypted for your privacy)
4. A nurse will call you when it is time for your appointment.
5. During your video chat, try to be in great light and with high speed wifi.
